Over the last four years our Primary Care Team has grown significantly and is able to provide the support that the patients of Eastleigh need

Mental Health Practitioners

MHPs support patients with a diagnosed or undiagnosed mental health concern.

They work across Eastleigh Health PCN to provide early assessment and treatment interventions for people with mental health illness.

They also provide additional signposting advice and information as needed.

Health & Wellbeing Coaches

Our Health and Wellbeing Coaches are experienced in mental health coaching.

This means the PCN can offer a unique specialised level of coaching support for patients facing mental health challenges.

The Coaches explore what their patients need and want, and how they can find healthy solutions.

Social Prescribing Services

Social Prescribers connect patients to community-based support that can meet the practical, social and emotional needs that can affect an individual’s health and wellbeing.

They can provide more time and focus on what matters to you. 

They take a holistic approach to health and wellbeing, empowering you to take control of your life.

Solent Mind Team

Wellbeing Assessors assess your current emotional difficulties and teach skills to retrain the brain’s reactions to events. They teach people about how to use our senses to switch off the 'survivor brain' and bring our thinking back online.

Peer Support Workers link the clinical side of support and real life.

Solent Mind are part of our Multi-Disciplinary Team (MDT).

How does the team work?

The team receives referrals from a variety of sources such as GPs, Nurses, other practice staff and community services. Our Social Prescribers accept self-referrals and can often be found hosting drop-ins out in the community.

These referrals are reviewed and allocated to the appropriate team on a weekly basis.

The team also have a weekly Multi-disciplinary Team (MDT) meeting on a weekly basis, where they can get guidance and support from their peers. 

Once a month the Community Mental Health Team (CMHT) and xxxxx join this meeting to create a more joined up link between your GP practice and the mental health team that is looking after you.
