I am the Team Lead Pharmacist specialising in Type 2 Diabetes and Cardiovascular Disease with 10 years of experience across community pharmacy, care homes and general practice. My extensive background in community pharmacy has allowed me to bridge the gap between local pharmacies and the surgeries, ensuring healthcare remains integrated and patient centred. I am committed to engaging with the wider community, raising awareness and supporting proactive health management initiatives.
In my role, I collaborate closely with the Multi-Disciplinary Teams to enhance patient outcomes, minimise medication-related risks and deliver Structured Medication Reviews. I am deeply passionate about empowering patients, helping them improve medication understanding, adherence and holistic care.
