I am the Social Prescribing Team Lead and Bereavement Counsellor. I joined the PCN in January 2021
during the Covid pandemic when it was hard to signpost patients to groups as there weren’t many running.
My experience includes working as a Hoarding Coordinator for Southampton City Council, in the housing department and a Care Manager at Southampton General Hospital Discharge Team.
