Introduce the team
- The Housebound Service works across all three practices in the PCN, they see very unwell housebound patients for urgent on the day appointments.
- The team is made up of two paramedics, a care coordinator – and they work closely with the Pharmacy Team.
- You may also see one of our team if you are housebound and need an annual review for a long-term health condition such as heart disease, asthma or diabetes.
- The housebound team also delivers the flu and covid vaccination to housebound and care home residents.
- If you feel you need an urgent home visit or proactive review please contact your GP
